What is a Medical Centre?
A medical centre is a community-based healthcare facility where an interdisciplinary team—family physicians, nurse practitioners, nurses, and allied health professionals—provides comprehensive, non-emergency care under one roof. Unlike hospitals (which deliver emergency and inpatient services), medical centres focus on prevention, diagnosis, treatment, and ongoing management for individuals and families.
At Openfield Medical Centre, this includes:
Primary care for all ages: routine check-ups and urgent concerns (as capacity allows)
Preventive services: immunisations, screenings, and risk assessments
Chronic disease management: structured plans for diabetes, hypertension, asthma, and more
Whole-person care: women’s and men’s health, paediatrics, mental health support, and minor procedures
Care coordination: referrals to trusted specialists, and navigation for labs and imaging
Access options: in-person and virtual visits, medication reviews, and planned follow-ups
Privacy & records: protection of your information under Ontario’s PHIPA
Coverage: most medically necessary services are OHIP-covered; we’ll advise on any non-insured services
How can I schedule an appointment?
You can book by calling the clinic during office hours. If calling after hours, please leave a message; we’ll return your call as soon as possible.
What should I bring to my appointment?
Please bring:
Your Ontario Health Card (OHIP)
A photo ID
A current list of medications (including doses) and any allergies
Relevant medical records or recent test results, if available
Can I be referred to a specialist if needed?
Yes. If your condition requires specialised care, our clinicians will provide a referral to trusted specialists and help coordinate your care.
Is my medical information kept confidential?
Absolutely. We protect your personal health information in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and our clinic’s privacy and data-security policies.
What should I do in a medical emergency?
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
For urgent but non-life-threatening concerns, please call the clinic to check if a same-day or next-day appointment is available.
